Unite Us Summit Brings Providers Together to Support Vulnerable Populations
ALBUQUERQUE - The Inaugural “Unite Us” Summit was held today in partnership with Bernalillo County, the City of Albuquerque, community leaders, and Unite Us staff. The summit brought together about 100 professionals, with 50 different organizations, including community service providers, government agencies, and healthcare providers, to network and learn about the new platform.
Unite Us creates a better support system for our unhoused and vulnerable communities. It connects healthcare providers, non-profits, and other key partners through a single system. This unified platform streamlines the continuum of care and simplifies the referral process, getting people connected to resources.
“Unite Us creates a continuity of care between providers, ensuring the client feels heard and at the center of an organization instead of walking through each door and having to tell their story repeatedly,” said Sarah Spain, Bernalillo County Transition Planning Services Manager.
At the summit, held at Los Duranes Community Center, providers registered for the platform, which will unleash new resources for the people they serve. Attendees learned about the platform, attended panel discussions, and networked.
“We are grateful to our partners and the community for making this inaugural summit a success,” said Health, Housing and Homelessness Deputy Director, Ellen Braden. “We are excited to be a part of a better, more streamlined continuum of care that serves our community.”
“This is a powerful example of our community working together to build a system that supports the most vulnerable,” said Mayor Tim Keller. “Through this summit, providers and nonprofits continue building a system of care that connects those in need to services.”
“We are gratified by today's strong participation and look forward to full implementation of the Unite Us platform,” says Bernalillo County’s Wayne Lindstrom Ph.D. “This collaborative effort is crucial to ensuring our most vulnerable residents have timely and effective care coordination."
The inaugural Unite Us Summit reached capacity within weeks of opening registration, so two additional summits will take place. We will release more information as it becomes available.
