City, County, and Community Leaders to Host Inaugural Unite Us Summit

On February 6th, Bernalillo County, the City of Albuquerque, and Community leaders will partner to host a summit highlighting a new platform, Unite Us, that is creating a better support system for the unhoused community and those experiencing housing or food insecurity. The Unite Us platform launched in September 2024 and connects healthcare providers, non-profits, and other key partners through a single system. Having a unified platform streamlines the continuum of care and simplifies the referral process for resources like food, housing, and behavioral health services.

The summit will take place from 11 a.m. to 3 p.m. at Los Duranes Community Center, 2920 Leopoldo Rd NW in Albuquerque.

The summit's goal is to connect more partners to Unite Us so that we can continue building the network and inform community members about its uses.

Through the Unite Us Platform, healthcare providers and community organizations can screen residents for needs and send referrals to appropriate organizations, just like your doctor’s office. Each agency can update the platform with outcomes, including what specific services were provided, so there is a more coordinated system of support and communication.

“The community requested this summit, and we are delighted to be a part of it to showcase how non-profits and service providers can be a part of this transformational system,” says Health, Housing, and Homelessness Deputy Director Ellen Braden. “The Unite Us Summit will serve as a space to learn how Unite Us benefits clients and to answer community partner questions.” 

“This is an example of government and community working together to build a system that ensures the most vulnerable in our community don’t fall through the cracks,” said Mayor Tim Keller. “Through the  summit, providers and nonprofits will continue building a coordinated system of care and more opportunities for folks to get connected to services.” 

"We know that collaboration and building strong community connections is the key to operating a successful closed-loop referral network,” says Bernalillo County Deputy County Manager of Behavioral Health, Wayne Lindstrom. “The Unite Us Summit is an opportunity for community service providers to learn more about the platform that will enhance the network of care in Albuquerque and Bernalillo County to ensure our most vulnerable members have continuous access to needed services. In addition, it will provide us with an opportunity to learn from these providers, what they need in terms of capacity and infrastructure, to not only implement this technology but also to actively utilize it for meaningful care coordination."
